so what i think happens is there is a portal that opened up at the bottom of the pool that the frigid girl disturbs when she sinks down there.

the demons try to corrupt her body, but don't have much luck, because she's so tense. so they infect the two lovers instead, who are preoccupied with fucking around.

The girl tries to escape by going through the portal at the bottom of the pool, but unwittingly crosses over into the demon dimension. There she is instantly killed by some eldritch horror.

Serious answer: Her mom could have used a little more folate in her diet very early in the pregnancy. Lack of folic acid can lead to spina bifada, where the cord isn't fully closed. She doesn't have that as any kind of real problem, but just enough for a cosmetic dimple above the anus. The dimple thing she has is something I've seen doing diaper changes on my own kids, but maybe it's rare. Not really seen it much in porn after all.
